FINANCIAL INFORMATION

Please be aware that it is the patient’s responsibility to inquire about any admission policies your insurance carrier may have. Failure to obtain pre-authorization, physician referral or a second opinion may greatly reduce or eliminate your benefits. If you are unaware of such policies, please call your healthcare provider and/or your physician’s office.

Ascent Surgery Center will contact you after completing your insurance eligibility to discuss any co-payment or deductible amounts due on the day of surgery. Please note that the facility fee does not include any additional costs. For example, but not limited to; laboratory, pathology, implants, surgeon or anesthesiologist fees. These fees will be an additional cost.

We will bill your primary and secondary insurance carrier or governmental agency for services rendered at the facility. If you do not have insurance, or you are a cosmetic patient, payment will be due prior to surgery.

Please bring your current insurance or Medicare card(s) and photo ID on the day of surgery for verification purposes.

Ascent Surgery Center accepts MasterCard, VISA, AMEX, personal* and cashier's checks.

Please contact Ascent Surgery Center at 469-986-3700 should you have any questions regarding billing prior to surgery or your patient statement.

*Note, a charge of $45 will be applied to patient accounts for all returned checks.
